On May 8th, 2024, my signature at the ER was forged on the consent form, without my knowledge or consent, by the person registering me. I have the printout from the computer with my name on it, but it isn't my signature.
Rose Cosme, of Patients Relations, has been doing everything in her power to suppress the truth and come up with lame, nonsensical retorts in order to quash my complaint. This woman couldn't care less about the patients, only about covering her butt and the hospital's butt.
Rose Cosme just told me over the phone that camera footage doesn't show anyone signing anything (wow--I guess the signature on the consent form appeared out of thin air), and in the same breath, after I asked her if the camera shows *me* signing anything (since I never signed anything even though my signature is right there on the consent form), she replied that the camera shows me standing near the desk signing something. I thought you just said the camera doesn't show anyone signing anything. Get your facts straight, lady.
To reiterate:
I was never handed any consent form to read.
I never signed any consent form.
A signature of my name is right there on the consent form.
Someone other than me signed the consent form.
Conclusion: The person at the registration desk forged my signature on the consent form at some point while I was waiting to be seen by a doctor.
I have filed a police report about the forgery.
This hospital is big and powerful, and I'm the little guy. They have already succeeded in removing one of my reviews on another site.
Beware of having your privacy and HIPAA laws violated by this hospital, and know that you will get zero help from them if you have a complaint against them.
You know, if they're like that when it comes to a forged signature, what must they be like if they screw up in even more serious ways and matters?

John Zaharis, Patient Access Director, called me today and told me they were unable to substantiate my "claim." Of course not. We wouldn't want your hospital accused of any wrongdoings, now, would we?
I said look at the camera footage, which Rose Cosme had mentioned. He said he wasn't aware of any camera footage. I said, well, look at it and see if it shows me being given the tablet with the consent form on it, reading it for a while, signing it, and then handing it back to the registration woman. It's very easy to do if you really want to.
The police officer that agreed with me the two signatures are nothing alike, and went ahead and filed the police report for me? John Zaharis said that's not necessarily true.
So then what *is* true, John Zaharis? I have a copy of the consent form with a signature on it that isn't mine, and I challenge you to find camera footage of me standing there reading and signing the consent form. But you won't look at the camera footage because you're not interested in finding out the truth, only in covering your aspic.
After listening to what he called "your side of the story" (I didn't know there was another side to this story of a forged signature appearing on my consent form), and reiterating, again and again, like the dutiful, obedient employee he's expected to be, about not being able to substantiate my claim, he told me he was closing the case.
He was closing the case. I said to him, "You know what? I'll be fine, but you're stuck with that awful woman, who forgot to give me the consent form, forged my signature on the consent form, and then lied about it, denying that she had ever done that. And this woman deals with gravely ill, injured, and wounded people at the registration desk at the ER and has access to all their most personal information." Good luck with that.
All John Zaharis had to say is, "I'm ending this conversation." I slammed my flip-phone shut.
Both Rose Cosme and John Zaharis are prime examples of people in powerful positions a little cog like me has no chance against, even when truth is on my side. What am I to do? They work for a powerful hospital that has slews of lawyers. They can do whatever they want with full impunity. Their loyalty is to the institution, not to the customers--patients, in this case. And they have a way of making corruption on their part go away.
Rose Cosme shamelessly lied throughout this entire thing. She is a truly reprehensible person.
John Zaharis is an unprincipled, spineless little man who will back up a lying employee with the aid and backing of his unethical superiors.
