My mom was at The pavilions at Sunny Hills for about three months. During this time, we have had top-tier service from all departments at the pavilion. Eve from Admissions was amazing, she was the one that I had talked to in the beginning of looking for nursing homes. She came highly recommend. When I came in for a tour, she greeted us with such a friendly disposition, super sweet and so nice. I loved her attitude. She made sure that, we were able to tour the facility and look at everything that we needed to look at. She explained everything that we needed to hear. When we walked in, it was so nice, quiet, clean and so welcoming. When we toured the physical therapy rooms and occupational therapy rooms, I was so impressed on how it was professional, clean, and so nice. We toured all upstairs and it was for long term care and it too, was pretty nice with big rooms. When we toured her potential room, that was my immediate yes. The solo suites were impressive and her bathroom was really nice. During her time there, Mariana really helped us from social services, she is so sweet and so professional with everything. She made sure that we had everything that we needed. Every question she answered and also helped us with receiving medical supplies on discharge. I would say occupational and physical therapy were top-tier. They really helped my mom with her rehabilitation and getting her strength back in her body. All of them were so sweet, funny and supportive. The nursing team was amazing. Any concern or any need that I had they were quick to answer and follow through with whatever was needed. If I felt that she was getting sick, or needed something to look at, they didn't hesitate to check on my concerns out. That made me feel at ease. Even when I would call a couple time or have questions, they were pretty easy to reach. I would say some days it was hard to get through, to talk to some departments, but most of the time it was easy. The CNA's, were also amazing. They were all super sweet and nice. They took extra care with showering, changing or whatever she needed. They kept her clean and moisturized. The lights were answered pretty fast from what I've seen. Sometimes they are busy, but they were answered pretty quickly for even being busy. The speech therapists were also so sweet and very helpful for my mom. The custodian department also did a good job in helping to keep my mom's room clean. We didn't go to activities so much, but it was really nice when we went. The food, I would say was OK. It was a hit and miss. But overall it was good and the options were good. Overall, my mom's experience was amazing, she felt heard and seen. She felt really cared for and I felt at ease, knowing that she was being cared for when I wasn't there. I would 100% recommend this place to anybody looking to have someone they love in a safe and good environment.

We visited a few other facilities before choosing this one, and we’re very glad we did. They offer private rooms (for an additional charge), and it’s absolutely worth it—especially if you’d like to stay overnight with your loved one. The staff is extremely friendly, attentive, and caring. I feel confident knowing my dad is in great hands here.

This is an excellent facility that used to be a hotel! Private rooms, kind staff, and great physical and occupational therapy! Special shout out to Eve, the Admissions Director, who coordinated the transfer of my mom from the hospital to The Pavillion and served as my primary liaison. Eve treated us with the utmost in compassion and respect! 5-stars, I highly recommend!!!

The staff at The Pavilion at Sunny Hills has been absolutely wonderful. Mercy and Victoria have shown such incredible kindness and compassion while caring for my father, and the lead nurses are attentive and professional as well. The therapy team has also made a huge difference for us. The physical, occupational, and speech therapists have been patient, encouraging, and deeply dedicated to my father’s recovery after his stroke. They not only work hard with him each day, but they also give us hope that he will regain his strength and return to the person he was before. Their positive energy, reassurance, and genuine belief in his progress mean the world to our family. We are so grateful for everyone here—from the nurses to the therapists—for treating our father with dignity, respect, and compassion during such a difficult time. This level of care and encouragement has made all the difference for our family.

This is my dad’s 2nd stay. One was back in May 2025. He shared with me how helpful and patient they are with him but also push him to do more which he likes and needs. He also said he’s very happy with the food and often give him some of his favorites. (Cottage cheese) which he loves! We are very thankful for The pavilion at Sunny Hills!

Everyone at The Pavilion took such excellent care of my Dad while he was there. They were so caring, patient, and attentive. They were excellent listeners and were always there to answer questions. I especially appreciated Alejandra and all the wonderful nurses, as well as E.J. and Mariana, the amazing social workers. This was my first visit to Sunny Hills. The staff I met were friendly and helpful. The corridors were clean and everything smelled nice. My sister's roommate insists on playing non-stop TV, and, even though it was turned off for a few hours at night, it was turned on again at 4am. (The only fault with Sunny Hills is that they don't control this more.) It also took them nearly 2 days to figure out her meds. (My sister has stage 4 cancer and high blood pressure. She definately needs her pain medication).

This facility is a mixed bag. My mother had a fall at home which resulted in a short hospital stay and subsequently was transferred here to The Pavilion for a three week rehabilitation stay. Here is my review. 1 Patient care here is inconsistent, hit or miss. The wait time for response to the call button can be LONG. There are some staff who appear to not care at all while others give quality, compassionate care. 2. The rehabilitation department, consisting for us, of physical therapy and occupational therapy is superior. There are plenty of quality therapists who are great with this patient population. They have two gyms with plenty of equipment and also sometimes work with patients in their rooms. If rehab is your primary emphasis I recommend this facility. 3. Food. My mother has no dietary restrictions so that menu is what I will address. The food here is like what Forrest Gump calls "a box of chocolates", you never know what you're gonna get. Sometimes it's pretty good other times not so much. We supplemented their food often with homemade food and I would suggest the same for anyone. 4. The support staff is quite good. Proficient at their jobs and compassionate. 5. Courtney at the front desk is a jewel. Always pleasant and helpful. 6. Cleanliness. This facility is well-maintained and always clean. I would give them five stars for cleanliness as well as maintenance. The maintenance staff even went above and beyond and when the brake broke on my mother's personal wheelchair. They went out of their way to help her out and repair it. Kudos to them.

The pavilion at Sunny hills is a great run business I was a patient at this location for a couple weeks if you want to go and I was treated really kindly and they always kept me clean and the food was awesome the CNAs for like totally cool everybody over there was considered and they always treated you good as a patient I would recommend this place to anybody that's needing a rehabilitation plus my wife is in this location as of today and I know this should be getting the right treatment as I did and is anybody else did they have a two rooms that are rehabilitate to get you back on your feet so the best location to go to.

The way this facility is portrayed online is not what I see in the 2nd and 3rd floor. It's super old, dingy and depressing! Some of the staff is unprofessional and mostly do the bare minimum. They are ran thin! The parking is terrible! The physical therapist and speech therapist are good and they have a complimentary coffee machine in lobby microwave available upon request. I don't know what other facilities look like but this one is NO BUENO!

Recently after a couple of days at Kaiser Hospital in Irvine, I was dispatched to this skilled nursing facility for further treatment. They provided physical and occupational therapy six days a week and I must say overall the level of care was excellent from the doctors to the nursing staff and here's something amazing even the food was delicious! The place was as clean as a whistle and the best part is the staff really care about the patients they want you to get better. Can't say enough good things about this establishment. No one wants to be in the hospital or in rehab, but I highly recommend this facility. They have all kinds of activities if that's your thingand there's plenty of places to walk inside the facility as well as the grounds as well as places to meet with your guests while you convalesce. If it was possible to give them seven stars, I would.

Care for my mom was good but then my mom is pretty nice. We are 6 WEEKS OUT from her arrival, and she is now back home now in AL, and we cannot get the people at Pavilion to FILL OUT and return insurance forms, after SIX WEEKS. If you don't have insurance I Guess you can go there but do not count on them to know what the devil they are doing when it comes to paperwork. When a 92 year old woman who cannot obviously take care of the paperwork side of such a thing cannot get this done for her no matter how much the family works for it, then this is not a suitable place for an elderly person.

My husband came to this facility following a surgery to repair a broken femur. He was unable to bear any weight on the injured leg and was at Pavilion for Physical Therapy to build strength. My spouse also deals with dementia made worse by the anesthesia from surgery. Rather than show him kindness, patience and respect, they began to ridicule him for his lack of recall. We left before our time was up and went home to get home health benefits. I don't recommend this facility. The aides were kind and responsive as was the nursing staff. Food was so so.

Just got discharged today after a 8 day stay. My 4th stay here. Every time I was here it never disappoints .nurses are helpful and caring. The physical therapist are the best. The facility is very clean.the food is ok. The breakfast is fair lunch was great every day, dinner was good. I recommend sunny hills for everyone

My father was at Pavilion for 6 months. (My mother spent 6 months here in 2019 and I knew this was the only skilled nursing facility we would choose.). Eve in admitting remembered us, facilitated my dad getting a great, private room, and worked hard to make the process as seamless as possible. The rooms are large and comfortable. The rooms, and entire facility, are kept very clean. You can do a white glove test anywhere here. The therapists are very good and administrative staff wonderful. The superstar for my dad was Elena Bello who cared for my father as if he were her own. Elena went above and beyond her many duties. My dad lost a lot of his appetite; Elena would bring him his favorite food. She rubbed castor oil on his pained abdomen. She kept him very clean. The list goes on. Elena is an absolute gem and we were so very blessed to have her caring for our dad. Pavilion is very lucky to have her.

The Pavilion is a full service Skilled Nursing Facility. Good for long term care or short term rehab care. They have a beautiful well equipped gym filled with equipment used in mobility & strength rehab, including a car! The Medical Care eam keeps pace with the excellence of their Physical and Occupational Therapy teams. The facility has an ongoing Social/Activities program with daily and weekly events from Coffee Socials to Themed Movie Nights with Dinner. The facility is well maintained and staffed, with the aim to help you achieve your health goals.

The staff here and care quality is amazing here. You can feel the love and professionalism and expertise here. We were here for 3 weeks and left for home just for thanksgiving. But unfortunately she fell again. We are now back again. Don Trinidad and staff is amazing. Really going above and beyond. It's so hard to care for loved ones and the pain is already hard enough to bare and deal with. The stress level runs high but these people are just angels sent to surround mom with love and care. I'm so eternally grateful for the services they provide. Really truly at the top of their game. I trust my mom's life with these people and am so impressed! Thank you

By far the worst skilled Nurseing facility I have ever seen. I was working here for night terrible indifferent staff. Didn't even bother warning me that one of the residents had covid. They spend way to much time talking amongst themselves instead of delivering proper care

I had a weeks stay at the pavilion at sunny hills Jan 2024. And feel like they did a great job taking care of my needs to get well enough to return home. The staff were very professional and took pride in the work they do. I managed to lose weight but was well feed and never felt hungry after a meal. I was lucky and my bed had a wonderful sunrise view over the neighboring golf course. I would highly recommend The Pavilion at Sunny Hills for anyone in need of post hospital rehabilitation.

I was at the Pavilion at Sunny Hills Nursing Skilled Facility for 9 months. The care that I received was phenomenal. The Nursing staff was very responsive, caring, and very professional. The Rehab that I received: Physical Therapy, Occupational Therapy, and Speech Therapy was outstanding. The Pavilion staff was very professional and was responsive to my requests. The Facility was clean, comfortable, and very pleasant. The room that I stayed in had great amenities and was clean and comfortable. I would recommend this facility to anyone needing to recover from any physical disabilities or illnesses.

My mom spent 2 weeks at the facility after she had a series of strokes. The facility was amazing. All of the staff were helpful and kind. I was very impressed with all of the nursing staff. Elena Bello was extraordinary. Elena was knowledgeable, kind and patient. She would speak to my mother with respect and address her by her name. Elena expressed a familiarity with her patients. I would recommend the facility for their care but Elena's exceptional care put the facility over the top.

I wish I could give this place a zero star. Giving a one star is too modest, given how unprofessional they have treated our situation. My grandmother was sent here, for rehab. The staff and doctors were very nice and helpful. My family and grandmother really enjoyed the care and service. Once my grandmother healed, she returned home. Unfortunately her condition worsened and she needed to return. For some reason my grandmother was "banned" and could not return to this rehab center. This was shocking news to my family and I. Apparently my family was "mean" to the staff and "were not allowing" the nurses to do their job. There was no discussion or warning from any staff members of this "behavior" nor any documentation to support their reasoning. The obvious goal of sending her to rehab is to heal and get better, therefore, why would my family be "mean" or "not allow" the staff to do their job? I come from a large, supportive family so I can speculate it has to do with the amount and/or frequency of the visitors but this is not a reason to ban anyone, especially without warning. It is unprofessional from this rehab center to instantly ban someone from treatment, especially with the lack of documentation and reasoning to support the ban. This action impacts and hurts my family tremendously. Now that she is banned, locating a rehab center has been challenging.

My father-in-law is at the pavilion. I feel they have poor communications in regards to answering the phone and also being able to talk to the patient that is there. I called one time. He said that he doesn't answer. I called the next time. I'm on the phone for a long long time with no response. I called again and nobody answers the phone. It's very frustrating when you have your love one there and you wanna communicate and see how they're doing. I can't even talk to the nurse because the nurse doesn't answer the phone. I would say that their system regards to their phone services and connecting with your loved. One is very poor. I would definitely ask them to approve that.

I give it a one-star because mom's doctor there refuses to treat her for her UTI with antibiotics. Therefore she is getting sicker & sicker. She's been untreated for over six weeks and today she said "I'm upside down, please help me!" She got to the Pavilion around August 12th and was diagnosed with a positive UTI at the ER just before she went into The Pavilion at Sunny Hills. Since the Pavilion at Sunny Hills refuses to treat her, the infection go to her brain and she hallucinates like crazy. Plus, by not treating her UTI infection it has gotten much worse and she is hallucinating daily. Doctor Pellegrino writes it off as "dementia" even though she doesn't have dementia. All he ever tells me is, "she doesn't have a UTI," even though the tests are positive and the cultures look terrible. I've had fits with the doctor and nurses over her mistreatment and the director of nursing told me that I can't call the nurses anymore since they don't have time for me. Now it is October 3rd and FINALLY a specialist that Pellegrino sent her too is forcing him to treat her with IV antibiotics. I hope this place doesn't kill her. P.S. Early September the Dept. of Health notified the Pavilion that mom has CRE so she is now being isolated. The director of nursing told me that mom had CRE before she came to them which is not true.

Absolutely worst facility on orange county when it comes to patient care. I would not recommend anyone send their family member to this location. Nurse Jessica has the worst bedside manner I have ever experienced. Everyone working seems to want to collect a paycheck, but no one seems to want to do their job. Absolutely horrible experience.

If I could I would give this place 0 stars. My grandma came her after having hip surgery. She was sitting up for 2-3 hours at a time and walking when at Kaiser. After being moved to the Pavilion they told her she needed to stay in bed. She started to have chest pain and tried calling for help. She tried multiple times to call for assistance with no response. She finally called my uncle to ask him for help. He called the rehab to inform them that my grandma needed help and then drove over which is at least a 30 min drive. They STILL had not gone to check on her. He then MADE them call 911 after they tried to talk him out of it. She had a heartache and needed emergency surgery. They could have killed her with their incompetence! This isn't even all the issues we had with this rehab and my grandma was there less then 24 hours.

DO NOT send your loved ones here they will neglect, talk bad about patients in the open nurses station leave used needles out in open for 2 days until I told them... tell you every excuse why they haven't done something leave patients in solid diapers. No communication they never call you back over give meds when given. Tell them not to give a certain med they say ok "take it off list" next nurse is there w same meds you told them not to give. Don't clean catheters you have to basically do their job for them cause they won't just do. Takes a person to go down to nurses station after button pushed for over 20 mins to get them off their phones to help. Bring your own food each meal cause food is not edible or what they say it is.

My father fell and fractured his hip and needed rehab before coming home. Having lived in YL since 78' all of his doctors and familiarity to the area would be key. But where to go? St. Jude had a few options but nothing was open at the moment. By chance there was a private room that just opened up and I hurried over and immediately was met by Carlos and knew that this would be where he needed to be. The hustle and bustle met with friendly attitudes and clean facility is what we wanted and got. Now my father demanded to go home every minute. He is very independent and modest. Set in his ways and did not want any help and was rather rude at times. I knew he was scared as he is the sweetest person in the world. But he still gave everyone a hard time. He refused to eat the food (fancy pants) and so I brought his food every day. He refused to let the nurses change his adult diaper or bathe him. And at times he would raise his voice to leave him alone. The moment I met his nurse Elena I knew she would not give up on him and she did not. Thank god for her caring heart and not taking it personal when my dad would get upset. She made sure I brought him food, bed was clean and did not give up on him. She even stayed after her shift was done because she knew he needed a shower and she would be off work for a few days and she had him in the shower with the physical therapist all clean until she returned. There was nothing she let slip by or had to be asked twice. I am so thankful for everyone at The Pavilion at Sunny Hills. All of his doctors and nurses were amazing and I attribute their hard work for where he is now in his recovery. I had a total left knee done and went there for basically therapy. The doctors, the nurses and the aids all the way to dietary were amazing. I cannot say enough good things about how wonderful they were physical and occupational therapy. Listen to my home situation and made adjustments in my care. And my CNA Elena was spectacular if I could give her 10 stars I would. She double checked because I have dietary restrictions every one of my meals. She went out of her way to make me comfortable and found out what I liked and made sure I had it at my meals she was just a ray of sunshine. They were all phenomenal. They seriously need to fix the kitchen cooks and menu system , literally no one here eats the food. They sneak it in. I will say this the nursing staff( Abby and Lovely Julia Chris and the nurse on second floor who spook Spanish) how they are not working for a top end hospital I will never know? and physical therapy unit are best I have literally the best I've ever seen The CNAs Kayla and Elena Steffi . . Mainly worked with Peter and Brandon with the exception of Ilene for OT ( I know I spelled that wrong. But the entire PT unit are probably some of the best PTs if not the best I have ever seen and worked with) The wound care people are top notch. Francis Janelle Gomez O'Dell and another wound nurse. One last thing, stop using money to build 2 gazebos and use it more wisely. Some other policies need to change. Stop having your administrative staff yell at nurses for the dumbest things. I literally saw it with my own two eyes. Could believe how bad this woman was yelling at a CNA for the dumbest things . Had it not stopped I would have jumped in and gave her piece of my mind. Want to reprimand someone do behind closed doors. You also never in someone's face. I have zero tolerance for this type power trip behavior. No wonder you have a high turnover rate. Not anyone's fault but your own.You run the ship Do Better If I hear of any backlash from this, I will report the SNF., to the proper departments for investigation.

We have been coming to visit a dear relative who is staying here after knee replacement surgery. She has been here two weeks and might be here a few weeks more. She has physical therapy twice a day and enjoys gaining more strength and range of motion. I am giving three stars because of the food, time spent waiting for tests, and the heavy accents the staff has-which make it difficult for a senior citizen to understand. My poor relative does not like the food at all!! It is bland and not what she normally eats. Last night for dinner there was limp chicken strips and beets with onions!! The tiny salad looked like grass clippings!! She was so disappointed. The decaf coffee is a Sanka packet!! We were also very disappointed as we waited far too long for an ultrasound on her leg to see if a blood clot was present. Over eight hours is ridiculous!! She got so anxious and angry waiting. At least this establishment is clean and well-maintained. There is a fancy coffee and cocoa machine in the lobby. There are also ample places to sit and visit other than in the patient's room.

Where to begin? If I could give 0 stars I would. This place seemed to get worse and worse throughout my grandmother's 3 week stay. My family started calling it Sunny Hell. The communication from the staff to the patient and patient's family is HORRIBLE! Good luck getting them to answer the front desk phone. You have to call repeatedly before anyone picks up. One time they finally picked up on the fourth call, but all I could hear was the staff laughing/goofing off and they hung up on me--TWICE! Also, they never call back or will "transfer" you and leave you on hold indefinitely. The worst thing was that there was a COVID outbreak and after my grandmother tested positive, it took them 2 full days to get the Paxlovid treatment to her. Worst of all was that they committed a CRIME by recording in the log that they had given it to her the day she tested positive, when they hadn't. They even LIED to the various family members that called to see if she had received the Paxlovid yet (my grandmother had told us she had not received it). This UNETHICAL and DANGEROUS CRIME of lying about medication that was not given (and going so far as to falsify medical records) was discovered by a Kaiser rep. There should be some serious repercussions for what they did! One night my grandmother told me she had not yet received her pills. When I called they said that the only person that had the key to the medicine storage had left, so they couldn't give any patients their meds until they could pry open the med storage. SERIOUSLY?! She did finally get her meds later that night. The food is so bad that grandmother could not eat most of it. I started getting doordash to deliver food because I was worried she was not getting enough nutrition. Each time I tried this was a nightmare. They would not bring her the food until hours after it was delivered and cold/unsafe to eat. Even when I called ahead of time and they assured me they would bring it to her right away, they left the food sitting for 2 hours. One time someone even ate the food I had delivered. Though they later sent someone to replace the chicken that went missing, my grandma didn't end up getting the food until 4 hours after it was initially delivered. In conclusion, this place is an ABSOLUTE DISASTER. They are unprofessional and uncaring. They neglected my grandmother's needs. I'd like to add that I was nice each time I called, but dealing with them was an extremely frustrating and stressful experience. I would not trust them to give proper medical care/treatment to anyone. DON'T SEND YOUR LOVED ONES HERE!

The people here are awesome, we came here after my mother's back surgery, and she needed to go back to the hospital, so they had to give away our room when mom was admitted back to hospital. There were no rooms available when we were getting ready to leave after a week, but Eve in admissions went above and beyond to get us a temporary room with an extra bed for me! We had several nurses who remembered us and rushed to greet my mother and say hi. I can't thank the staff enough for everything they have done to help us out. Note, the kitchen makes my mother's 2 eggs perfectly every morning. Thanks to Eve and Nick for going the extra mile. We are happy to be back

I've had two relatives stay here and I highly recommend. First rate care from all staff members throughout. Pristine facility with excellent occupational and physical therapy, frequent doctor rounds, nurses provide exceptional, gentle care and food quality is outstanding.

Don't send your loved one here. The administration is unprofessional and extremely hard to get a hold of. Call the only listed number and no one answers. Where did the receptionist go during business hours? Call the number, talk to the receptionist, ask for a case or business manager, their voicemail box is full. Ask to leave a message and no one calls back. My calls were time sensitive, involved insurance questions or care related. After 9 days of zero communication I took a day off of work to visit the facility. The person I needed was conveniently not there. When money was due my phone received 5 calls in 2 days. The actual nursing staff and support team were fantastic but very short staffed. I waited 45 minutes outside, in my car, for someone to bring out a wheelchair for my mother. I reminded the receptionist that we were waiting and she acted like she forgot and said they were short staffed. My mother fell during the end of her stay, resulting in multiple staples in her head and major skin tears. She was sent to ER. I called to get information and no one was available to discuss what happened. Accidents happen. The communication experience makes me very concerned. If you need to get a hold of your loved one make sure they have a cell phone close. The phone they provide is often physically out of reach.

When my mother-in-law needed skilled nursing, we were able to visit her at The Pavilion several times a day for the week that we were in town. At every visit, we were able to touch bases with her caregivers, observe the physical and occupational therapies, check out the meals, talk to the cleaning staff etc. We were impressed in every instance! We were especially impressed with Elena Bello, her nurse, who kept us updated with her treatment, and filled our every request for special meals, rescheduling therapy, etc. Elena did everything she could to make my mother-in-law as comfortable as possible. We were able to return home, knowing that our loved one would be cared for with expertise and tender care!

My mom came to The Pavillion after hospital stays for chronic UTIs over a 6 month period. Honestly, I dreaded having to send her to another Skilled Nursing Facility, as the places she'd been to before were a nightmare. She'd been to Skilled Nursing Facilities 4 times in 6 months and had been in worse condition after each stay, coming home with the same infection she'd had when she was admitted to the hospital. I could go on and on about the horrid conditions of the other nursing facilities, but it would result in an extremely long review. With the exception that it sometimes takes a very long time for the front desk to answer the phone, I've never had a negative experience with The Pavillion. From admissions, to her social worker, Mariana, to her charge nurse, Sophia, everyone there has been outstanding. The staff are always friendly and accommodating. And the care she receives is so far above the other facilities that there really is no comparison. My mother has always had high expectations of care when it comes to anyone who is ill or incapacitated. When she was at the other facilities, she constantly complained about the lack of care she received, and I could see the attitudes of the staff were completely lacking in compassion. While she's been at The Pavillion, I've asked her every time I visit, and every time I call, if she is being treated well. Every single time she tells me they're taking good care of her, and that she's happy there. I'm so grateful and relieved that I found The Pavillion. My mother will never leave there, because she's too stubborn to do the physical therapy that has been recommended. I know this is true because she did the same thing when she lived with me. It's heartbreaking that she's made that choice, because before she became ill, she was vibrant and independent. The wonderful thing, though, is that I never have to worry about her now. There've been a lot of comments about the quality of the food, and I'll admit the food is not amazing, but I wouldn't expect it to be. It's not a gourmet restaurant. It's a wellness facility, and they absolutely provide wellness care. The food is also not horrible, and it's definitely better than any of the other facilities where she stayed. It's obvious that they're trying. Personally, I think people who are expecting great food in a nursing facility are unrealistic. Also, the facility is always clean, and there is never an unpleasant odor there. I wouldn't hesitate to put a loved one there again, if it became necessary, and that's high praise coming from me. Thank you, to everyone there, for putting my mind at ease and helping me feel comfortable that my mother is being well taken care of and is obviously cared about. Without hesitation, I 100% recommend The Pavillion.

I've been waiting to review because I didn't know exactly how to put it into words. I was there recovering from major surgery and was much younger than the average resident/patient. My review: the good things were very, very good. The "bad" things were bad. Not much middle ground. Good: morning & afternoon nursing staff. Shout out to Jasmine! Some of the PT/OT staff. The wound care team was exceptional. Bad: the food. The food. The food. It was totally lacking of fresh fruits and vegetables.The night staff, while I was there, had a hard time with English. Some of the PT staff was very unprofessional. Lack of parking. Good: Wes, the Director, heard my concerns and seemed to run the place well.

As a Geriatric Care Consultant I have visited more than 40 different skilled nursing facilities and if I had to choose one for my loved one or myself The Pavilion at Sunny Hills would be in my top three choices. There are many reasons for this: 1) It is a very clean facility with no odors. They have a dedicated cleaning staff and I regularly see them cleaning the common areas, rooms, and hallways. 2) Quality physical therapy and occupational therapy. I have observed more than a handful of my clients receive therapy in one of their two physical therapy rooms Unlike other rehab centers Sunny Hills has large rooms allowing the patients an open and safe area to work on gaining their strength and independence. They therapist are first class! The positive energy and words of affirmation they give to their patients is uplifting and plays a crucial part in motivating their patients. It is easy to see that they love their job. 3) The Case Managers and Social Workers are skilled and thorough. If you get a chance to choose your social worker I highly recommend Shiela Correa. Shiela is a professional that knows her stuff! She has the tools and resources to make sure her patients are discharged with the support they needs. She coordinates doctors appointments, verifies all medications are ordered, and takes the time to explain everything to her patients and their families. Thank you Shiela for making my job easier and for taking such great care of your patients and my clients. I appreciate you! 4) Their Administrator Wes is available to families and comes with a wealth of experience. It is not common to families and patients to have direct access to the Administrator or a skilled nursing facility. It says a lot about the leadership of Sunny Hills to have access to the boss of the facility. 5) The medical director of Sunny Hills is the best in the industry. Geiss Med is a mobile physician that is well respected in Southern California. He regularly does rounds and his office is easy to communicate. Thank you Pavilion at Sunny Hills for making a positive different in the lives of your patients and their families. We need more SNF's like you!

They were horrible! They just leave the medication while the patient is sleeping and do not monitor when they take it. They then give it again at the scheduled time and almost killed my sister. They are rude to the patients and do not bathe them regularly. If you ask, they will make faces at you and treat the patients like kids. Very sad that this place is still open. If St Jude recommends them, do not go. Beware of sales pitch

We have been fortunate enough to get a bed at Pavillion through Kaiser several times over the past several years and I have nothing but praise for the facility. I think most people will agree that any stay at a nursing facility is not on your bucket list. If the time comes for some needed rehabilitation, this is the place. They have an extensive menu with alternative choices for meals. Granted some of the meals could have been warmer, but in the overall scheme of things are we really after gourmet dining or great medical care. The place is clean and well maintained with limited parking. The staff is wonderful and very helpful and professional. They are staffed similar to other locations which is not the same as a hospital. We never had a problem with getting help when we needed it. Our experience at the Pavillion has been outstanding

Like others, I wish there was a zero star option. I can't stress enough to never, ever consider this place for your loved ones. If I could sue them I would. My beloved Godfather died here. Where to begin. When my Godfather was admitted they did not screen people for COVID. They did not ask any visitors to show proof of vaccination or at the very least show proof you are COVID free. My Godfather had special circumstances as he was a dialisis patient. He shared a room with a person who had guests. We were told this guest/s were the reason he had COVID. He died because of this incompetence. Before I found out he specifically had COVID , I would visit. I would get text updates that there were patients getting COVID. Yet still, at entry ,no COVID free ,proof ,reveal to staff or proof of vaccination. Why?? You are sadly having a Covid outbreak at a facility that specifically deals with elderly people who are not in condition to fight it if they were to get it. When they called me and told me he had it. I became angry. I went off on the person in charge . They told me he will be alright. Sadly he was not. They seemed annoyed with me for being upset? Like my anger was unwarranted. Are you kidding? Let's talk about the nursing staff. Most were Cold, aloof,self involved female dogs. They were annoyed because he was "Needy". Now my Godfather may not have been the easiest to deal with but dealing with elderly and stubborn personalities is part of your job! You aren't working there for free or out of the goodness of your heart, you are getting a healthy pay check. Shut up and deal with it! He was very thin so he got cold a lot. He would ask for more blankets etc.. My GFather 's body was weak but his mind was extremely cognizant. He would tell me they took a long time getting to him. He was on an appetite stimulation medicine so when he ate he could eat frequently but had to go to the bathroom a lot. He would ring for them but they wouldn't come in time and then get annoyed when they did come and find that he soiled the sheets. I would call or visit and I knew the service he was getting was not good. When I would call for updates they didn't seem pleased and made Sure to tell me that he was needy or in a bad mood that day etc. Why are you telling me this? You are getting paid to deal with this and patients like him. Do your job! Although I am happy for other patients, I am devastated for my Godfather because after his death when I came to get his belongings, I finally saw a, you need proof of being Covid free sign in the lobby before entering the facility. This should have been the rule all along you idiots! By the way, I had to make two trips. Why?? Because they gave me his ROOMATES belongings! You see he died in the Covid unit. I was allowed to stay with his body until the funeral company showed. I was devastated and not thinking about his belongings. So when I was leaving I told staff I forgot to get his belongings. I wasn't allowed back into the unit to get his belongings from his closet. I had to rely on staff. Staff only gave me a little bit of his belongings but more of his roommates. I was so devastated I just took the bag and didn't check. When I went back to return his roommates belongings and to retrieve the last of his you should've seen the looks on their faces when I had to tell the staff what happened. They had "oh crap !"look on their face! Like I previously stated, if you love and value your loved one you would never ,ever, consider this place. I pray it closes someday. If you have no choice but to put your loved one in there because it's a Kaiser facility and there's no other beds available in any other facility I beg you to stay with your loved one as much as you can. Be on top of it at all times. Also please read Krystal K's review below mine, here. She states things that I experienced but didn't mention here such as NEVER or minimal phone contact.

I was sent there post-op. Every single day, I had to correct something about my medication (and sometimes remind them to give it to me). Once there was a pill that I did not recognize and asked what it was. The tech said oh they look different from different companies. While that is true, I insisted she go find out what the pill was. A few minutes later, she came running into my room, grabbed that pill and ran out without saying a word. One particular medication was prepared wrong every day. This in spite of me giving them the instructions each time (which they could have read on the container, but never did). I caught Covid there just before I would have been discharged to home. So they moved me to a so-called isolation wing for 10 days. They refused to follow my doctor's orders regarding care of my incision. He talked to them on several occasions because I told him that it wasn't being done. Still they wouldn't do it, tho promised the doc they would. They had to call nursing registry in order to have a Charge Nurse on duty. They refused to let me bathe/shower, and I was there 17 days. (Actually, I did shower after I was strong enough but I had to do it on the sly and without a shower chair.) The bed linen was not changed during my stay. They use a cloth, not plastic, blood pressure cuff...and told me not to worry because after all, they sterilize it once each shift. I told her she must sterilize it between each patient and I wanted her to do it before it touched me. With a big sigh and roll of the eyes, she set it on my bed, and with a baby wipe, took a few swipes on the pump, not the cuff! The place is filthy, the employees do not wash their hands, and don't even use the hand sanitizer (it was empty the whole time that I was in isolation.) I took wet paper towels and wiped the spots in my room that I couldn't avoid touching (door knobs, hospital tray, remote controls etc) and ended up with a pile of really gross-looking blackened and gooey paper towels. I did this both in my regular room and in the isolation room. I was able to advocate for myself quite a bit, but none of the other patients I met there would have had the ability to prevent medication errors, or clean their own room, or know they could insist on clean equipment. Some were even non-verbal like my roommate. I tried to advocate for her, but they kept saying she was fine...until they had to call 911 and send her to the hospital! (Later I learned that she actually had a different infection than I had...yet we were placed in the same "isolation" room!) The people, except one, were nice, some especially so. The one that wasn't was doing the work of 3 or 4 people, so I rather understand why. I feel a little bad about having to say bad things about the facility when the people were nice. They just need a lot more training and help.

My Mother-in-law was here in rehab before she passed away. She described this place as "jail." Her husband recently was released. He told us that he was pushed, shoved, mishandled, and spoken to without respect. Maybe it was him fighting ? But there shouldn't be fighting. How about some more help for visitors who cannot feed themselves ? They became under-nourished. How about a vitamin drink ??? It's a "One flew-over-the-cookoo's nest" scenario with residents yelling out for help, and in pain. Are the meds correct ? As both were in la-la land while there. I know that a resident needs to want to get better, but I feel like more help should have been given.

My father recently had a three-week stay at The Pavilion after a hip fracture. Since it was his first time in a skilled nursing facility, I can't speak to how it compares to other operations, but I can say that my family had a positive experience during a stressful and frightening time. The biggest draw was the fact that my dad could have a private room, which was cleaned twice daily. He was already very anxious about his injury, and the word "nursing" set him off, but being able to have his own quiet space helped a lot. My mom was able to stay with him 24/7 thanks to the pull-out bed the Pavilion provided. Another plus was that, at least when it came to the ground floor, the space did not have that sour smell you sometimes find in other nursing facilities. The staff in general is quite accommodating. Chab, who helped with getting my dad admitted, was kind and reassuring. Rigo, one of the room attendant, gets five stars. My dad's physical therapist Maria was very nice, as was Nick, the director of physical therapy. Big thanks to Joanna Gomez, who handled the social work piece and was always available to explain things when I had questions. It wasn't perfect. Inevitably, challenges and miscommunications came up because there were so many moving pieces, but I was grateful that even when that happened, there was accountability on the part of the staff. We're grateful to everyone at The Pavilion for helping us get through my dad's rehabilitation.

A BAD PLACE !!!!! NOBODY SEEMS TO KNOW WHAT'S GOING ON IN THIS HIGHLY QUESTIONABLE FACILITY. THE NURSE TO PATIENT RATIO IS 2 NURSES TO 30 PATIENTS. St Elizabeth is far better it's 600 feet north of the Pavillion 2800 N. Harbor blvd. I wouldn't recommend the Pavillion at sunny hills to your worst enemy. because on the evening my wife arrived they said that they had oxygen sensors on every patient at all times . Not true. My wife used the call button and it took over a half hour to answer the call button. The second time she called for the nurse it took an hour and a half , In no way is this a good place . Recommend it to your enemies not your loved ones.

The staff here are great! From before we checked in, to the moment we left, everyone cared for us like family. The food was just ok, but so is any larger organization. The key is attitude of the staff is always friendly and attentive. The building used to be a hotel. My mom, being a short-term rehab patient, had a large hotel-like room to herself. The other places I have visited had patients sharing rooms half the size. This place is not for everyone. It's not a high-attention environment like a hospital. However, if the expectation is that this is a transition from hospital to home, then it can be an excellent place for recovery.

Honestly, we cannot say enough about this place. Typical Kaiser run facility. Spot On! The care and therapy my mom has received is superb. They are wonderful communicators. Everyone is so nice. Place is spotless, every room and patient well cared for, and here is the kicker - When you go, the halls are filled with people walking and getting therapy. Their goal is to get you back to your life. Very grateful for these people. As many patients as they have, they keep track and keep everyone going. We had begun to think that mom would always require 24hr care, but we now think that she stands a fairly good chance of going back to assisted living. And if she does not, it will be because of her own choices, not because she did not get what was needed at Pavillion. Pavillion is giving her the best chance possible of regaining her independence. Very grateful to have found this place.

D not leave you loved one here!! My father was here for about a week. The first night we used his call button and it took them over 30-40 min to answer. We were assured when touring the facility that there was a 3-minute turnaround time for the call button. Lies! I asked the nurse why it took so long and she said "everyone is on their break". Seriously?! Turns out my father was anxious his first night alone there. He told me they put him in his wheelchair and wheeled him out to the nurses station so they could "keep an eye on him". Who does this?!! Patients are tired and weak and instead of letting them have their rest, they get them up out of bed and leave them sitting in a wheelchair for who knows how long...TOTALLY UNACCEPTABLE!! I left my dads room that night at 10pm so who know what time they got him out of bed!! The nursing staff didn't even inform me of this incident, my dad was the one who told me about it. When I saw my dad early the next morning, he had a look of fear in his eyes that I had never seen before. He said he did NOT want to stay there. We immediately started looking at other facilities. The following morning I went to see him very early in the morning. His nurse approaches me and says my dad feel out of bed?! What?! My dad is paralyzed on his right side...how can he fall out of bed?! I found out the bed alarm was not put on his bed even though the staff was trying to tell me it was. If it was, then why didn't the alarm sound? They tried to tell me he might have disconnected it somehow on his way down to the floor. Are you kidding me?! I told the nurse to show me how he might have disconnected it...she couldn't even disconnect it with her own two hands! On top of all this, they were giving him his insulin incorrectly. His insulin needed to be given right BEFORE meals. They were giving it to him 1.5-2 hours before meals when he has nothing in his system. Totally careless!! The nurses here should have their license revoked!! There are so many more negative things I can say about this place, but I'm sure by now, you get and idea of how this place runs. After making a huge deal about the treatment here, they offered my dad a private room on the 1st floor. The care here was much better. I don't know if this is because the patient to nurse/CNA ratio is smaller or if it's because of all the complaining we did. The only good thing I have to say is that my dads therapists were great. His speech therapist, physical therapist and occupational therapist made my dad feel very comfortable and the Nurse Practitioner he saw, I believe her name was Asia, was very nice and caring. If I had to do it over, I WOULD NOT put my dad in this facility. They are highly understaffed and unprofessional. If your family member needs lots of care and assistance, this is NOT the place for them. They will neglect your loved one. Consider this your warning!!
