REVIEW ON CARE RECEIVED FROM UMCSN FOR MY SISTER:
October 12, 2023, Thursday - My sister was brought UMC Urgent Care in Summerlin because she was complaining of pain on the neck and shoulder and that she was not feeling well. The attending physician at the Urgent Care prescribed her the following medication on that day: Acyclovir, Baclofen, Gentamicin and Naproxen. The Nurse Practitioner wrote the prescription for Baclofen 20 MG 1 TABLET TO BE GIVEN BY MOUTH EVERY 6 HOURS AS NEEDED FOR MUSCLE SPASM.
October 12 - October 20, 2023 - From the Urgent Care she was transported to UMC Hospital located on Charleston Blvd. They continued giving her Baclofen during her stay at the hospital.
October 20 - November 10, 2023 - She was transferred to Las Ventanas in Summerlin for a 3-week rehabilitation. They continued administering Baclofen to her at the rehab center.
November 10 - November 17, 2023 - She was home recovering and went to her regular dialysis schedule. On Friday, Nov 17th when she got home from dialysis she was complaining of pain on the neck. She had dinner, took Baclofen for the pain and went to bed.
November 18, 2023 (Saturday) - Around 9 AM they tried to wake her up so she can eat breakfast and take her medication, but she couldn't get up and she was very weak and disoriented. She could not eat and was just laying down not looking well. In the afternoon they called 911. The paramedics took her to UMC Hospital on Charleston Blvd. Her siblings followed to the ER and stayed there till midnight. The attending physician at the ER told them that they could not see anything wrong with Rosario. Baclofen was administered to her during her stay at this hospital.
November 19, 2023 (Sunday) - When her siblings visited her early morning, they were surprised to see her intubated and in the ICU. The hospital performed dialysis on her at a normal speed for 4 hours. They told our siblings that they will put her on her regular dialysis schedule.
November 20, 2023 (Monday) - She was intubated at the ICU.
November 21, 2023 (Tuesday) - She was intubated at the ICU.
November 22, 2023 (Wednesday) - I think it was on this day when the doctor visited her at the ICU when her niece in Chicago requested the doctor to test the level of the Baclofen's toxicity in her system and the doctor said that it will be of no use at that point since it was already flushed out during the dialysis.
November 23, 2023 (Thursday-Thanksgiving Day) In the morning around 9 AM a sibling talked to a nurse in the ICU and asked if they can do something about the tube because she was having a hard time with it. After a couple of hours, the sibling talked a male doctor, and this doctor clearly said to the sibling that they should not have given the Baclofen to her and the nurse was there listening.
November 24, 2023 (Friday) - She was taken off the tube and was moved to a different ICU room.
November 25, 2023 (Saturday) - She was still lying in the ICU.
November 26, 2023 (Sunday) - She passed away around 8:45AM.
There was a difference of judgement in administering Baclofen to her at the hospital's facilities. The doctor at the Urgent Care prescribed it but the doctor at the ICU stated that she should not have been given Baclofen.
Her family had an urgent request for the hospital's administration to seriously look into this matter and to take strong measures to ensure that this wrong prescription/dosage which was fatal NOT happen to another patient in the future and reprimand the responsible parties.
After 11 days of receipt of our email, this is all they can say:
"After review of your sister's medical record, I am unable to find documentation that you were her designated Power of Attorney (POA) or delineated as next-of-kin. Due to Federal patient privacy HIPAA) laws, I am unable to discuss the specifics of your sister's care with you. However, as with all concerns regarding the quality of care and services rendered by UMC, an investigation is conducted by staff from the Center for Quality & Patient Safety and associated department leaders. This includes a review of the medical record and discussion with members of our healthcare team.
Our sincerest condolences to you and your family. We are grateful for your feedback which assists us in improving our care and services. As of the date of this correspondence, UMC considers this matter to be closed."
The patient's family does not find any closure from the response.
